Conduct initial telehealth assessments for older adults and clinically complex members enrolled in a care pilot, confirming medical needs, social context, barriers to care, and near-term clinical priorities. The physician serves as the clinical entry point for a longitudinal, team-based program and helps shape the care plan that community health workers, social workers, and care navigators implement over time. This role emphasizes whole-person, relationship-driven care, with fully virtual visits and EMR documentation.
Key Responsibilities
- Conduct comprehensive telehealth initial visits for newly enrolled members.
- Synthesize complex medical and social histories to identify clinical priorities and risks.
- Create concise clinical plans and handoffs for community health workers, licensed clinical social workers, and care navigators.
- Document findings and recommendations clearly in the EMR to support ongoing care.
- Provide clinical guidance on escalation pathways and pilot program workflows.
- Build rapport and trust with members and caregivers during virtual encounters.
Required
- MD or DO degree.
- Active, unrestricted New Mexico and/or Pennsylvania medical license.
- Board-certified or board-eligible in Family Medicine, Internal Medicine, Geriatrics, or a related primary care specialty preferred.
- Substantial experience with Medicare/Medicaid populations or complex chronic disease management.
- Experience with telehealth assessment and virtual rapport-building.
- Strong clinical judgment and comfort working with complexity and ambiguity.

- Experience in primary care specialties serving older adults or complex patients.
- Interest in care coordination and collaborative, whole-person care models.
