Provide initial virtual clinical evaluations for older adults and other complex patients, focusing on medical needs, care gaps, barriers to care, and near-term care priorities. The role is centered on setting direction for each member’s care journey rather than managing a traditional longitudinal panel. Ongoing engagement is supported by care navigators, Community Health Workers, and Licensed Clinical Social Workers, with opportunities to help refine workflows during a pilot program.
Key Responsibilities
- Conduct initial telehealth visits with enrolled members
- Assess medical needs, care gaps, and barriers to accessing care
- Establish a clear clinical plan and care priorities
- Collaborate with care team members to support handoff and continuity of care
- Provide clinical input to refine workflows during the pilot phase
- Support a patient-centered approach that addresses social and medical needs together
Required
- MD or DO degree
- Active, unrestricted New Jersey medical license
- Board certified or board eligible in Family Medicine, Internal Medicine, or a related primary care specialty preferred
- Comfort working with Medicare, Medicaid, or complex populations
- Strong communication skills and ability to build trust quickly in a virtual setting
- Interest in care coordination, health navigation, or whole-person care models
