Conduct initial virtual visits with older adults and other complex members to assess medical needs, care gaps, barriers to care, and health goals. Use those visits to establish a clinical plan and set priorities, while a community-based care team handles ongoing follow-up and engagement. This role centers on telehealth, care coordination, and whole-person care in an early-stage, evolving program.
Key Responsibilities
- Conduct initial telehealth visits with enrolled members
- Assess medical needs, care gaps, and barriers to accessing care
- Establish a clear clinical plan and care priorities
- Collaborate with care team members to support a smooth handoff and continuity of care
- Provide clinical insight to help refine program workflows during the pilot phase
- Support a patient-centered approach that addresses social and medical needs together
Required
- MD or DO with an active, unrestricted license
- Board certified or board eligible in Family Medicine, Internal Medicine, or a related primary care specialty
- Comfort working with Medicare, Medicaid, or other complex populations
- Strong communication skills and ability to build trust quickly in a virtual setting
- Interest in care coordination, health navigation, or whole-person care models
- Comfort working in a startup-like, evolving program environment
Preferred
- Multiple state licenses
