This physician role focuses on initial virtual visits with older adults and other complex populations, assessing medical needs, care gaps, barriers to care, and overall health goals. The clinician establishes an initial care plan and priorities, then hands off ongoing support to a team that helps with navigation and follow-through.
Key Responsibilities
- Conduct initial telehealth visits with enrolled members.
- Assess medical needs, care gaps, barriers to care, and health goals.
- Establish a clear clinical plan and care priorities.
- Collaborate with care team members to support continuity of care.
- Provide clinical insight to help refine workflows during the pilot phase.
- Support a patient-centered approach that addresses social and medical needs together.
Required
- MD or DO with an active, unrestricted medical license.
- Board certified or board eligible in Family Medicine, Internal Medicine, or a related primary care specialty.
- Comfort working with Medicare, Medicaid, or complex populations.
- Strong communication skills and ability to build trust quickly in a virtual setting.
- Interest in care coordination, health navigation, or whole-person care models.
- Comfort working in an evolving, startup-like program environment.
Preferred
- Multiple state licenses.
- Telehealth experience.
