Conduct remote audio or video intake visits with Medicare patients to understand care-navigation needs and determine whether they are appropriate for patient advocacy services. The role focuses on assessing barriers to care rather than diagnosing or prescribing, with work centered on a startup model that supports Principal Illness Navigation and Community Health Integration. You also collaborate with a technology team to help shape AI tools that support the clinical workflow.
Key Responsibilities
- Conduct 20–30 minute telehealth intake appointments by audio or video.
- Assess patient challenges and determine suitability for patient advocacy services.
- Coordinate with patient advocacy workflows that help patients access appointments, transportation, equipment, and related needs.
- Collaborate with the technology team to improve AI-enabled clinical tools.
- Use a fast-paced, startup-oriented approach to support evolving workflows.
- Communicate clearly with Medicare-aged patients and support teams.
Required
- Board certification in family medicine or internal medicine.
- Active medical licensure in an IMLC member state, with openness to obtaining additional licensure.
- PTAN/PECOS registration and enrollment with Medicare.
- Experience working with Medicare patients.
Preferred
- Prior telehealth experience.
- Comfort using technology and learning new tools quickly.
- Experience in an early-stage startup environment.
- Strong communication and interpersonal skills.
