Conduct audio or video intake visits with Medicare patients, focusing on seniors who need help navigating access to care and determining whether they qualify for patient advocacy services under Principal Illness Navigation and/or Community Health Integration. The role centers on assessment and care-navigation triage rather than diagnosis or prescribing. It also involves close collaboration with a technology team to help develop AI tools that support the clinical workflow.
Key Responsibilities
- Conduct 20- to 30-minute patient intake appointments by audio or video.
- Assess patients’ care-access challenges and support needs.
- Determine whether patients are appropriate for patient advocacy services.
- Coordinate with the technology team to improve clinical tools and workflows.
- Support a care-navigation model for Medicare-aged patients.
- Use strong communication and interpersonal skills in patient interactions.
- Work in a fast-paced startup environment with flexible weekly availability.
Required
- MD or DO degree.
- Board certification in Family Medicine or Internal Medicine.
- Active licensure in an IMLC member state and willingness to obtain additional licensure as needed.
- PTAN/PECOS Medicare enrollment and registration.
- Experience working with Medicare patients.
- Availability for at least 20 hours per week.
Preferred
- Prior telehealth experience.
- Comfort learning new technologies quickly.

- Strong interest in patient advocacy and improving care access for older adults.
