Conduct remote audio or video intake visits for Medicare patients, primarily seniors, to understand their needs and determine whether they are appropriate for patient advocacy support under Principal Illness Navigation and/or Community Health Integration. The work is nonprescribing and nondiagnostic, with a focus on care access, navigation, and identifying barriers to care. The role also includes close collaboration with a technology team on AI tools that support the clinical experience.
Key Responsibilities
- Conduct 20-30 minute telehealth intake appointments by audio or video.
- Assess patient needs and determine suitability for patient advocacy services.
- Coordinate with patient advocates and support care navigation for Medicare patients.
- Collaborate with the technology team to improve AI-enabled clinical workflows.
- Work in a fast-paced startup environment with a flexible, evolving scope.
- Use strong communication and interpersonal skills to engage older adults.
Required
- MD or DO with board certification in Family Medicine or Internal Medicine.
- Active licensure in an IMLC member state and willingness to obtain additional licensure as needed.
- PTAN/PECOS enrollment for Medicare.
- Availability for a minimum of 20 hours per week.
Preferred
- Prior experience caring for Medicare patients.
- Prior telehealth experience.
- Comfort using technology and learning new tools quickly.
- Interest in startup environments and patient advocacy.
