Conduct telehealth visits with older adults to assess wellness goals, provide lifestyle medicine counseling, and build personalized, evidence-based care plans focused on prevention and healthy aging. Support patients across nutrition, movement, sleep, stress, social connection, and reducing risky substances, while anchoring care in age-friendly principles and coordination with existing primary care teams. Help shape the clinical model through protocol development, intake design, outcome tracking, documentation, and patient experience improvements, working closely with a behavioral health team in a remote, telehealth-native setting.
Key Responsibilities
- Conduct telehealth visits with older adults focused on prevention, behavior change, and healthy aging.
- Provide lifestyle medicine coaching around nutrition, physical activity, sleep, stress management, social connection, and substance reduction.
- Build personalized, evidence-based care plans aligned with patient goals and age-friendly care principles.
- Coordinate with patients’ existing primary care providers rather than functioning as the primary care clinician.
- Collaborate with behavioral health clinicians to support whole-person care.
- Help develop clinical protocols, intake workflows, documentation, and outcome measures.
- Use a patient-centered approach grounded in the 4 M’s of Age-Friendly care: What Matters, Medication, Mentation, and Mobility.
Required
- Active U.S. licensure and Nurse Practitioner licensure with Full Practice Authority in good standing, or an MD/DO degree.
- Board certification in Geriatrics, Lifestyle Medicine, or Family Practice for physicians.
- Experience caring for older adults.
- Comfort practicing via telehealth.
- Training, board certification, or strong working knowledge in lifestyle medicine.
Preferred
- Board certification or active pursuit of certification through DipACLM or DipIBLM.
- Multi-state licensure or willingness to obtain additional state licenses.
- Experience helping patients make sustainable behavior changes.
- Experience contributing to new or evolving care models.
