The clinician provides longitudinal palliative and serious-illness support for oncology patients and their caregivers, with a focus on goals-of-care conversations, advance care planning, symptom assessment, and end-of-life discussions. Care is delivered primarily by phone, email, and video, with urgent clinical escalations and coordination across the member’s broader care network as needed. This role sits within a value-based, tech-enabled oncology care model that emphasizes collaboration with nurses, medical directors, payers, and providers.
Key Responsibilities
- Conduct goals-of-care, values, advance care planning, and palliative care conversations with members and their care network.
- Provide longitudinal support to members with serious illness, including those near end of life.
- Conduct comprehensive symptom assessments and address clinical concerns through remote communication channels.
- Coordinate care and provide clinical support in collaboration with the broader care team.
- Participate in case conferences to review member progress and develop targeted support plans.
- Manage urgent clinical escalations and provide clinical consult support as needed.
- Collaborate with non-clinical care team members to address social needs such as food access and transportation.
- Maintain documentation standards and follow care team policies, procedures, and clinical protocols.
Required
- Active, unrestricted compact RN license.
- ADN or BSN degree in Nursing.
- At least 5 years of nursing experience.
- At least 3 years of experience in outpatient palliative care and/or home hospice caring for oncology patients and caregivers.
- OCN certification at hire or commitment to obtain it within 2 years of hire.
Preferred
- BSN degree.
- New York, Illinois, and/or Michigan licensure.
- CHPN certification.
- Oncology-related certification such as OCN, AOCN, AOCNS, or CCM.
- Fluency in Spanish, written and verbal.
