The clinician provides longitudinal palliative care support for people with cancer and serious illness, focusing on goals-of-care conversations, advance care planning, symptom assessment, and end-of-life support by phone, email, and video. The role also manages clinical escalations, coordinates with members’ care networks, and helps address practical needs such as transportation and food access. This is a tech-enabled, value-based care setting with close collaboration across nursing leadership, medical directors, payers, and providers.
Key Responsibilities
- Conduct goals-of-care, advance care planning, and palliative care conversations with members and their care networks.
- Provide longitudinal support for members with serious illness and, when appropriate, at end of life.
- Perform comprehensive symptom assessments and respond to clinical issues and urgent escalations.
- Communicate with members by phone, email, and video as part of remote care delivery.
- Collaborate with nurse leaders, medical directors, and the broader care team on protocols and best practices.
- Participate in case conferences to review member progress and coordinate targeted support plans.
- Coordinate with non-clinical team members to address social needs such as food resources and transportation.
- Document care accurately and follow care team policies, procedures, and quality standards.
Required
- Active, unrestricted compact Registered Nurse (RN) license.
- ADN or BSN in Nursing.
- At least 5 years of nursing experience.
- At least 3 years of outpatient palliative care and/or home hospice experience caring for oncology patients and caregivers.
- OCN certification at hire or commitment to obtain within 2 years.
Preferred
- BSN degree.
- Licensure in New York, Illinois, and/or Michigan.
- CHPN certification.
- Oncology-related certification such as OCN, AOCN, AOCNS, or CCM.
- Fluency in Spanish, written and verbal.
