Provide telephonic oncology care for members with cancer, focusing on clinical assessments, symptom monitoring, medication reconciliation, and support during transitions after admissions or other care changes. The role also includes discharge coordination, referral support, virtual home safety evaluation, and education for members and caregivers across the oncology care continuum. Care is delivered in a tech-enabled, value-based model with close collaboration across clinical, payer, provider, and non-clinical care teams.
Key Responsibilities
- Conduct telephonic clinical assessments, including pain assessment and medication reconciliation.
- Monitor for changes in health status and help members remain safe in the community.
- Coordinate care transitions, including post-admission follow-up and discharge planning.
- Collaborate with hospital case managers and providers to support follow-up care and reduce readmissions.
- Provide education and support to members and caregivers about diagnosis, treatment, and care needs.
- Perform virtual home safety evaluations and assess need for DME or supplies.
- Refer members to PT, OT, skilled nursing, palliative care, hospice care, and other appropriate services.
- Communicate member and provider feedback to clinical leadership and assist with administrative projects as needed.
Required
- Active, unrestricted compact RN license.
- Willingness to obtain additional state licensure as needed.
- ADN or BSN in Nursing.
- At least 5 years of nursing experience.
- At least 3 years of oncology nursing or case management experience.
- Certified Case Manager (CCM) credential.
- OCN certification at hire or commitment to obtain within 2 years of hire.
Preferred
- BSN degree.
- Spanish fluency, written and verbal.
- Experience with video chatting, Google Suite, Slack, or electronic health records.
- Experience working in fast-paced, rapidly changing environments.
