Provide telephonic oncology care coordination and clinical support for people with cancer, including assessments, symptom and pain review, medication reconciliation, and guidance for members and caregivers as they move through treatment and transitions. The role also supports discharge planning, readmissions prevention, referrals to supportive services, and escalation handling across the oncology care continuum. Care is delivered remotely or in a hybrid model, with close collaboration among clinical, payer, provider, and non-clinical care team members in a value-based, tech-enabled setting.
Key Responsibilities
- Conduct telephonic clinical assessments, including pain assessment and medication reconciliation.
- Provide education and support to members and caregivers about diagnosis, treatment, and care transitions.
- Coordinate care management after admissions, including discharge planning with hospital case managers and follow-up with providers.
- Monitor member progress and communicate updates during case conferences and care team reviews.
- Partner with payers, providers, and non-clinical care team members to address social determinants of health needs.
- Perform virtual home safety evaluations and assess the need for DME and supplies.
- Provide referrals to PT, OT, skilled nursing, palliative care, hospice care, and other supportive services as appropriate.
- Handle clinical escalations, urgent needs, and documentation according to care team standards.
Required
- Active, unrestricted compact RN license.
- Willingness to obtain additional state nursing licenses as needed.
- ADN or BSN in Nursing.
- At least 5 years of nursing experience.
- At least 3 years of oncology nursing or case management experience.
- Certified Case Manager (CCM) credential.
- Oncology Certified Nurse (OCN) certification at hire or commitment to obtain within 2 years.
Preferred
- BSN.
- Spanish fluency, written and verbal.
- Experience with video chat, Google Suite, Slack, or electronic health records.
