This role supports people with cancer through phone, email, and video outreach, providing triage, clinical assessment, education, and escalation management across the oncology journey. It focuses on solid tumor oncology and includes support from screening through survivorship or end-of-life care, along with care coordination, symptom support, and referral guidance. The work is part of a value-based, tech-enabled care model with close collaboration across nurses, medical directors, providers, payers, and nonclinical care team members.
Key Responsibilities
- Conduct clinical assessments and triage for members with cancer across the care continuum.
- Provide education, support, and symptom management guidance by phone, email, and video.
- Coordinate care needs, including discharge planning, provider referrals, and navigation support.
- Facilitate end-of-life care discussions and support members and caregivers through difficult decisions.
- Participate in case conferences and collaborate on targeted support plans with the care team.
- Communicate with nurse leaders, medical directors, payers, and providers to align on protocols and best practices.
- Address social needs such as food resources and transportation by working with non-clinical team members.
- Use documentation standards and care team workflows to support quality and efficient operations.
Required
- Active, unrestricted compact RN license.
- Nursing degree (ADN or BSN).
- At least 5 years of nursing experience.
- At least 3 years of solid tumor oncology nursing experience.
- Oncology-related certification such as OCN, AOCN, AOCNS, or CCM, or obtained within 2 years of hire.
Preferred
- BSN.
- Compact plus single-state licensure in New York and/or Illinois.
- Spanish fluency, written and verbal.
