This role provides oncology nursing support for people with cancer by phone and email, with a focus on symptom management, clinical triage, education, and care coordination across the cancer journey. The nurse conducts comprehensive assessments, helps manage urgent escalations, and supports members from screening through survivorship or end-of-life care. The work is highly collaborative, partnering with nurse leaders, medical directors, providers, payers, and non-clinical care team members in a value-based, tech-enabled model.
Key Responsibilities
- Conduct clinical assessments, triage concerns, and support symptom management for members with cancer.
- Provide education, guidance, and empathetic support to members and their care networks by phone and email.
- Coordinate care needs across the oncology continuum, including referrals, nutrition support, discharge planning, and follow-up needs.
- Respond to urgent clinical escalations and provide clinical consult support as needed.
- Participate in case conferences and collaborate with nurse leaders and medical directors on care plans and clinical protocols.
- Document member interactions and maintain policies, procedures, and quality standards.
- Collaborate with providers, payers, and non-clinical team members to address social needs such as food access and transportation.
- Support members from screening through survivorship or end-of-life care, including discussions related to end-of-life planning.
Required
- Active, unrestricted compact Registered Nurse (RN) license.
- Degree in Nursing (ADN or BSN).
- At least 5 years of nursing experience.
- At least 3 years of solid tumor oncology nursing experience.
- Oncology-related certification such as OCN, AOCN, AOCNS, or CCM, or obtained within 2 years of hire.
Preferred
- BSN.
- Compact plus single-state licensure in New York and/or Illinois.
- Fluency in Spanish, written and verbal.
