Provide phone- and email-based oncology navigation for people with cancer, focusing on triage, symptom management, education, and longitudinal care coordination across the cancer journey. Conduct clinical assessments, help manage escalations, and support care from screening through survivorship or end-of-life care. Work closely with nurse leaders, medical directors, payers, oncologists, primary care providers, and non-clinical care team members in a value-based, tech-enabled model.
Key Responsibilities
- Conduct comprehensive clinical assessments and monitor member health throughout the oncology care continuum.
- Provide triage, symptom management, education, and clinical support by phone and email.
- Coordinate care planning, provider referrals, discharge planning, nutritional support, and other member needs.
- Participate in case conferences and collaborate on targeted support plans with the care team.
- Support urgent clinical escalations and provide clinical consult support as needed.
- Collaborate with oncology and primary care providers, payers, and non-clinical team members to address care coordination and social needs.
- Maintain accurate documentation and follow care team policies, procedures, and clinical protocols.
- Engage members and their care network with empathy, active listening, and ongoing education.
Required
- Active, unrestricted compact Registered Nurse (RN) license.
- Degree in Nursing (ADN or BSN).
- At least 5 years of nursing experience.
- At least 3 years of solid tumor oncology nursing experience.
- Oncology-related certification such as OCN, AOCN, AOCNS, or CCM, or ability to obtain within 2 years of hire.
Preferred
- BSN.
- Compact and single-state licensure in New York and/or Illinois.
- Fluency in Spanish, written and verbal.
