Lead transitional case management for oncology members navigating high-risk care transitions, including hospital discharge, post-acute care, and movement between care settings. Provide clinical guidance on complex or escalated cases, collaborate with frontline navigators and care teams, and contribute virtual visits with members when needed. Help shape protocols, education, and technology-enabled workflows that support safer transitions and reduce avoidable utilization.
Key Responsibilities
- Drive the clinical vision and ongoing development of the transitional case management program.
- Conduct virtual visits with members as part of transition support.
- Provide clinical guidance for complex discharge planning, care coordination, and post-acute needs.
- Participate in case conferences and partner with oncology APPs, nurse navigators, and community health workers.
- Develop and refine clinical protocols, escalation pathways, and care transition workflows.
- Support staff education, training sessions, and ongoing feedback for the frontline care team.
- Track program performance and contribute to efforts to reduce avoidable readmissions and emergency utilization.
- Represent the clinical team in meetings with payers, health systems, and post-acute partners.
Required
- MD or DO with completion of fellowship training in medical oncology.
- Board certification in medical oncology.
- At least 3 years of clinical experience caring for oncology patients after fellowship.
- Unrestricted medical license.
- Willingness to obtain additional state licenses as needed.
Preferred
- Experience with transitional care management programs.
- Experience with care transition protocols.
- Experience reducing avoidable readmissions or emergency utilization in oncology or complex care populations.
- Comfort using video visits, EHRs, Google Suite, Slack, and other new technologies.
