Provide telephonic oncology care management for adults with high-acuity cancer needs, including clinical assessments, symptom and pain checks, medication reconciliation, and education for members and caregivers. Support care coordination across the oncology continuum, including admissions, discharge planning, referrals, and urgent clinical escalations, with a strong focus on safe transitions and culturally competent care. The role is remote or hybrid and works within a tech-enabled, value-based care model that emphasizes collaboration with payers, providers, and the care team.
Key Responsibilities
- Conduct telephonic clinical assessments for members with cancer, including pain assessment and medication reconciliation.
- Monitor changes in health status and identify priorities to help members remain safe in the community.
- Coordinate care transitions, including admission follow-up, discharge planning, and provider communication.
- Provide education and support to members and caregivers about diagnosis, treatment, medications, and care plans.
- Coordinate referrals and support for medications, appointments, PT, OT, skilled nursing, palliative care, hospice care, DME, and home safety needs.
- Participate in case conferences and develop targeted support plans with the healthcare team.
- Use clinical tools and documentation standards to manage escalations, tasks, and quality expectations.
- Collaborate with non-clinical team members, payers, and providers to address social determinants of health and reduce readmissions.
Required
- Active, unrestricted compact Registered Nurse (RN) license.
- Willingness to obtain additional state RN licenses as needed.
- Nursing degree (ADN or BSN).
- At least 5 years of nursing experience.
- At least 3 years of high-acuity adult oncology experience.
- Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) certification.
Preferred
- BSN degree.
- Spanish fluency, written and verbal.
- Experience working remotely.
- Comfort using video chatting, Google Suite, Slack, and electronic health records.
- Ability to obtain OCN certification within 2 years of hire, if not already certified.
