In this role, the clinician provides telephonic support for people with cancer, focusing on high-acuity adult oncology assessments, symptom monitoring, medication reconciliation, and care coordination across the oncology care continuum. The work also includes discharge planning, referral support, virtual home safety checks, and helping members navigate urgent clinical escalations, transportation, food access, and other social needs. This is a tech-enabled, value-based care role that relies on collaboration with members, caregivers, payers, providers, and the broader care team.
Key Responsibilities
- Conduct telephonic clinical assessments, including pain assessment and medication reconciliation.
- Monitor changes in member health status and help keep members safe in the community.
- Coordinate care transitions, including post-admission care management and discharge planning with hospital case managers and providers.
- Provide education to members and caregivers about diagnosis, treatment, medications, and follow-up care.
- Perform virtual home safety evaluations and assess the need for DME and supplies.
- Coordinate referrals to PT, OT, skilled nursing, palliative care, hospice care, and other services as needed.
- Collaborate with payers, providers, and non-clinical team members to address social determinants of health needs.
- Document clinical interactions accurately and follow escalation pathways, communication standards, and quality expectations.
Required
- Active, unrestricted compact Registered Nurse (RN) license.
- Willingness to obtain additional state licenses as needed.
- ADN or BSN degree in Nursing.
- At least 5 years of nursing experience.
- At least 3 years of high-acuity adult oncology experience.
- Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) certification.
Preferred
- BSN degree.
- Spanish fluency, written and verbal.
- Experience working remotely.
- Comfort using video chat, Google Suite, Slack, electronic health records, and other new technology.
- Commitment to obtain OCN certification within 2 years of hire, if not already held at hire.
