Provide longitudinal nursing support for people with cancer who have complex behavioral health needs, using telemedicine and telephonic outreach to assess symptoms, monitor progress, and help coordinate care. The role also includes medication reconciliation, pain assessment, education for members and caregivers, and escalation of urgent concerns. Care is delivered in close collaboration with social work, community health workers, and other members of an interdisciplinary oncology care team in a value-based, tech-enabled model.
Key Responsibilities
- Conduct telephonic clinical assessments, including pain assessments and medication reconciliation.
- Provide ongoing nursing support for members with behavioral health concerns ranging from adjustment-related coping needs to severe mental illness.
- Monitor member progress, document updates, and develop targeted support plans with the care team.
- Coordinate care, referrals, and access to medications and appointments.
- Partner with social work and community health workers to address social determinants of health such as food, transportation, and support at home.
- Participate in interdisciplinary case conferences, huddles, and pod meetings to align care plans.
- Route urgent or complex issues to the appropriate care team members or leaders and support clinical escalations.
- Maintain timely documentation, queue management, and adherence to clinical protocols and workflow standards.
Required
- Bachelor of Science in Nursing (BSN).
- Unrestricted Registered Nurse (RN) compact license with willingness to obtain additional state licenses as needed.
- At least 5 years of nursing experience.
- At least 2 years of experience using advanced clinical judgment in complex behavioral health cases.
Preferred
- Oncology nursing experience.
- Oncology Certified Nurse (OCN) certification, or willingness to obtain it within 2 years of hire.
- Experience with video chatting, Google Suite, Slack, and electronic health records.
