Provide virtual rheumatology care for adult patients with autoimmune and inflammatory conditions, including consultations, evaluations, diagnoses, treatment planning, and medication management. This role focuses on longitudinal follow-up for established patients through an established telehealth platform, with close collaboration with rheumatologists and other care team members. The work is fully remote and includes patient education, diagnostic review, and documentation in the EHR.
Key Responsibilities
- Conduct virtual rheumatology consultations, evaluations, diagnoses, and treatment plans.
- Manage chronic autoimmune and inflammatory conditions, including medication management and treatment monitoring.
- Conduct follow-up visits and longitudinal care for established patients.
- Collaborate with rheumatologists and other care team members to support continuity of care.
- Order and interpret diagnostic studies as clinically appropriate.
- Document patient encounters accurately and efficiently in the EHR.
- Provide patient education and counseling on diagnoses, treatment options, and lifestyle modifications.
- Maintain compliance with telehealth regulations, licensure requirements, and practice standards.
Required
- Active California license as a Nurse Practitioner or Physician Assistant in good standing.
- At least 2 years of rheumatology experience in outpatient or telehealth settings.
- Strong communication and clinical documentation skills.
- Ability to practice autonomously in a remote care environment.
- Reliable internet connection and familiarity with telemedicine technology.
Preferred
- Experience with telehealth delivery in a specialty care setting.
- Prior experience managing adult rheumatology patients in ambulatory care.
