Provide psychiatric consultation for a collaborative care team supporting primary care patients with behavioral health needs. The work centers on case review for patients who are new to care or not improving as expected, with medication and treatment recommendations to help guide primary care management. You also support care transitions for patients needing more intensive or specialized mental health services and offer education to the broader team.
Key Responsibilities
- Provide regularly scheduled caseload consultation to Behavioral Health Care Managers.
- Review patients who are new to the caseload or not improving under the current treatment plan.
- Recommend medication changes and other treatment interventions through case review or ad hoc consultation.
- Advise on care transitions for patients who need more intensive or specialized mental health treatment.
- Support the treatment of complex patients in the medical setting until specialty care is established.
- Conduct occasional ad hoc telephone consultation with primary care providers for challenging cases.
- Work with the care team to monitor caseload status and population health.
- Provide educational support on diagnosing and managing behavioral health conditions.
Required
- Licensed Psychiatrist or Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC).
- Active North Carolina license.
- Experience in a Collaborative Care Model (CoCM) or equivalent.
- Understanding of collaborative care delivery in primary care settings.
- Ability to synthesize medical and psychiatric information into evidence-based recommendations.
Preferred
- Dual Family Nurse Practitioner (FNP) certification.
- Experience collaborating in a team-based behavioral health setting.
- Strong communication skills.
