Provide remote telemedicine care for patients with HIV, hepatitis C, tuberculosis, PrEP needs, and other infectious disease-related conditions. This role centers on virtual evaluation, treatment, follow-up, and consultation, with additional support for innovative HCV treatment workflows and care coordination in a community health setting. The physician collaborates closely with interdisciplinary teams and uses electronic records, telehealth tools, and evidence-based protocols to support underserved patients.
Key Responsibilities
- Conduct telemedicine visits for evaluation, diagnosis, treatment, and follow-up care.
- Provide remote clinical consultation for HIV, HCV, TB, PrEP, and related infectious disease care.
- Coordinate care with nurse practitioners, primary care providers, care managers, pharmacists, and other team members.
- Review laboratory results, diagnostic testing, and specialty referrals.
- Document patient encounters and findings accurately in the electronic medical record.
- Respond to patient messages, clinical inquiries, and care coordination requests.
- Participate in virtual case reviews, interdisciplinary discussions, and provider consultations.
- Adhere to clinical guidelines, telehealth standards, HIPAA, and regulatory requirements.
Required
- MD or DO degree.
- Active New York State medical license.
- DEA registration.
- Board certification.
- Experience in HIV care and medication-assisted treatment (MAT).
- Ability to work with a diverse patient population in a community health setting.
