Conduct virtual wellness visits for adult and/or pediatric patients in the home setting, focusing on health assessments, preventive care, medication review, and cognitive screening. The role also includes care coordination, referrals, documentation, and collaboration with in-home clinical team members during patient visits. This position uses mobile and HIPAA-secure technology and emphasizes autonomous, patient-centered care.
Key Responsibilities
- Conduct virtual wellness visits for patients in their homes.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Provide education on health and well-being during preventive visits.
- Coordinate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Manage care coordination, including referrals to specialists and community resources.
- Document clinical findings and support medication management.
- Use laptops, mobile devices, and secure messaging tools for charting and coordination.
Required
- Board certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in California.
- Credentialed Medi-Cal provider in California.
- Ability to pass a national background check and clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.
Preferred
- Prior experience performing wellness visits.
- Technology-savvy and comfortable using digital charting and secure messaging tools.
- Experience working autonomously in a fast-changing or problem-solving environment.
