Conduct virtual wellness visits for adult and pediatric patients in the home setting, focusing on health assessments, medication reviews, cognitive screening, and preventive care. The role also supports care coordination, referrals, documentation, and collaboration with in-home clinical team members for services such as vital signs and blood draws. This is a tech-enabled mobile care model that emphasizes autonomous work, strong communication, and personalized patient engagement.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Coordinate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Provide preventive care interventions, patient education, and wellness guidance.
- Coordinate referrals to specialists and community resources.
- Document findings and manage care using secure digital tools.
- Work independently while supporting collaborative patient care.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in TX, LA, or OK.
- Ability to pass a national background check and valid clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.
Preferred
- Prior experience performing wellness visits.
- Technological comfort with laptops, mobile devices, and HIPAA-secure messaging tools.
