Provide virtual wellness visits for patients in their homes, with a focus on adult and/or pediatric preventive care, health assessments, medication reviews, and cognitive screenings. The role also includes care coordination, referrals, documentation, and support for low-acuity in-home services in collaboration with on-site team members. Clinicians use mobile and HIPAA-secure tools to manage schedules, charting, and care coordination while working independently and as part of a team.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in the home setting.
- Provide health risk assessments, medication reviews, cognitive screenings, and patient education.
- Coordinate care with in-home clinical team members during visits.
- Arrange blood draws, evaluate vital signs, and support other patient-specific tasks.
- Coordinate preventive care interventions, medication management, specialist referrals, and community resources.
- Document clinical findings and use HIPAA-secure messaging and mobile devices for care coordination.
- Review daily caseloads and prepare needed technology and tools before visits.
Required
- Board certified Family Nurse Practitioner.
- Active Family Nurse Practitioner license in North Carolina.
- Experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.
Preferred
- Experience working in a tech-enabled or mobile care environment.
- Comfort with last-minute problem solving and independent decision-making.
- Prior experience providing personalized preventive care.
