The clinician conducts virtual wellness visits for adults and/or children in patients’ homes, focusing on health assessments, risk screening, medication review, and preventive care. The role also supports care coordination, referrals, and basic in-home clinical workflows with a mobile team, using technology for charting and secure communication. This is a highly autonomous, tech-enabled care model built around home-based visits and collaboration with in-home clinical staff.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in the home setting.
- Perform health risk assessments, medication reviews, cognitive screenings, and patient education.
- Coordinate care with in-home team members to evaluate vital signs and arrange blood draws.
- Provide preventive care interventions, medication management, and referrals to specialists.
- Document clinical findings and care plans in electronic tools.
- Use laptops, mobile devices, and HIPAA-secure messaging apps for charting and care coordination.
- Work independently while collaborating with clinical team members as needed.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in TX, LA, or OK.
- Experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
- Strong written and verbal communication skills.
Preferred
- Prior experience in a mobile, virtual, or home-based care model.
- Comfort working in a fast-changing environment that requires problem-solving and flexibility.
