This role provides virtual wellness visits for adult and pediatric patients in the home setting, with a focus on health assessments, medication review, cognitive screening, and preventive care. The clinician also supports care coordination, referrals, and documentation while working alongside in-home clinical team members and using mobile technology to manage visits.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Evaluate vital signs and coordinate blood draws with in-home clinical team members.
- Provide patient education on health and wellness during the visit.
- Coordinate preventive care interventions, medication management, and specialist referrals.
- Document findings and use HIPAA-secure messaging tools for care coordination.
- Work independently while partnering with in-home team members to address patient needs.
Required
- Board certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
Preferred
- Experience working in mobile, virtual, or tech-enabled care settings.
- Comfort using laptops, mobile devices, and secure messaging apps for charting and coordination.
- Strong written and verbal communication skills.
- Ability to work autonomously and problem solve in a fast-changing environment.
