Conduct virtual wellness visits for adult and pediatric patients in the home, with a focus on health assessments, preventive care, medication reviews, and cognitive screening. The role also includes care coordination, referrals, documentation, and collaboration with in-home clinical team members to support low-acuity services and diagnostic workflows. This position is tech-enabled and requires independent work, strong communication, and a patient-centered approach.
Key Responsibilities
- Conduct virtual wellness visits and health assessments for adult and pediatric patients.
- Provide preventive care interventions, medication reviews, and cognitive screenings.
- Collaborate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Coordinate referrals to specialists and community resources.
- Document findings and support care management activities.
- Use digital tools for charting and HIPAA-secure care coordination.
- Deliver patient education focused on health and well-being.
Required
- Board certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Ability to pass a national background check and clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently and in a team environment.
