The Nurse Practitioner conducts virtual wellness visits for adult and pediatric patients in their homes, with a focus on health assessments, preventive care, medication review, and cognitive screening. The role also includes care coordination, referrals, and documentation, while working alongside in-home clinical team members to support home-based services such as vital sign review and blood draw coordination. The position uses technology for scheduling, charting, and HIPAA-secure communication in a mobile, on-demand care model.
Key Responsibilities
- Conduct virtual wellness visits for patients in the home setting.
- Provide health risk assessments, medication reviews, and cognitive screenings.
- Coordinate care with in-home clinical team members to review vital signs and arrange blood draws.
- Educate patients on health and well-being and reinforce preventive care interventions.
- Arrange referrals to specialists and community resources as needed.
- Document findings and manage care using digital charting and secure messaging tools.
- Work independently while collaborating with team members to address patient needs.
Required
- Board certified Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Georgia.
- Prior experience performing wellness visits.
- Ability to pass a national background check and clinical license search.

- Experience working in a mobile or home-based care environment.
- Comfort using laptops, mobile devices, and HIPAA-secure messaging apps.
- Strong written and verbal communication skills.
