Provide virtual wellness visits for adult and/or pediatric patients in the home setting, with a focus on health assessments, risk screening, medication review, and preventive care. Work closely with an in-home clinical team to coordinate blood draws, vital signs, referrals, education, and documentation while managing care through a tech-enabled workflow.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Coordinate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Provide preventive care interventions, patient education, and health and well-being guidance.
- Manage care coordination, including referrals to specialists and community resources.
- Document findings and use secure digital tools for charting and communication.
- Work independently while collaborating with the broader clinical team as needed.
Required
- Board certified Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Texas, Louisiana, or Oklahoma.
- Experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
- Strong written and verbal communication skills.
Preferred
- Experience working in a mobile, home-based, or virtual care model.
- Comfort using laptops, mobile devices, and HIPAA-secure messaging tools.
- Ability to problem-solve independently in a fast-changing environment.
