This role involves conducting virtual wellness visits for adult and/or pediatric patients in their homes, with a focus on health assessments, medication review, cognitive screening, and preventive care. The clinician also supports care coordination, referrals, and documentation while working with an in-home clinical team and using digital tools to manage visits and communicate securely. The setting is tech-enabled and home-based, with an emphasis on autonomous work and personalized patient care.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in the home setting.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Evaluate vital signs and coordinate in-home tasks with clinical team members.
- Provide preventive care interventions, patient education, and wellness support.
- Coordinate medication management, specialist referrals, and community resources.
- Document findings and communicate with the care team using secure digital tools.
- Use laptops or mobile devices to chart and manage care coordination workflows.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Experience performing wellness visits.
- Strong written and verbal communication skills.
- Ability to work independently and in a team environment.
- Pass a national background check and valid clinical license search.
Preferred
- Experience working in a virtual or mobile care model.
- Comfort with HIPAA-secure messaging apps and digital charting tools.
- Ability to solve problems quickly in a changing clinical environment.
