Key Responsibilities
- Conduct virtual wellness visits for adult and pediatric patients in the home setting.
- Provide health risk assessments, medication reviews, and cognitive screenings.
- Coordinate with in-home clinical team members to evaluate vital signs and support patient needs.
- Arrange blood draws and assist with diagnostic and low-acuity services as needed.
- Provide preventive care interventions, referrals to specialists, and connections to community resources.
- Document clinical findings and care coordination activities.
- Use mobile devices, laptops, and secure messaging tools for charting and communication.
Required
- Board certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in California.
- Credentialed Medicaid provider for California (Medi-Cal).
- Prior experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
Preferred
- Experience working independently in a mobile or virtual care model.
- Strong comfort with technology, including charting and HIPAA-secure messaging tools.
- Experience supporting both adult and pediatric patient populations.
