This Nurse Practitioner role focuses on virtual wellness visits for adults and/or children, with in-home patient interaction supported by a mobile clinical team. The work includes health risk assessments, medication reviews, cognitive screenings, preventive care, referrals, and documentation. Clinicians use technology to manage schedules, chart, and coordinate care while working closely with in-home team members.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Evaluate vital signs and coordinate blood draws with in-home clinical team members.
- Provide preventive care education and health and wellness guidance.
- Coordinate referrals to specialists and community resources.
- Document findings and manage care using laptops, mobile devices, and secure messaging tools.
- Work independently while collaborating with the in-home clinical team.
Required
- Board certified Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Experience performing wellness visits.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.
- Pass a national background check and valid clinical license search.
Preferred
- Prior experience in a technology-enabled or mobile care environment.
- Comfort with charting on laptops or mobile devices and using HIPAA-secure messaging apps.
- Experience working with both adult and pediatric patients.
