The clinician conducts virtual wellness visits for adults and/or children in the home setting, with a focus on preventive assessment, medication review, cognitive screening, and patient education. The role also supports care coordination, referrals, and documentation, while working closely with in-home clinical team members during patient visits. The workflow is tech-enabled and emphasizes autonomous practice, professionalism, and personalized care.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Collaborate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Provide preventive care interventions, patient education, and health coaching.
- Coordinate medication management, specialist referrals, and community resources.
- Document clinical findings and communicate care needs using secure digital tools.
- Work independently while maintaining high-quality, patient-centered care.
Required
- Board certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Experience performing wellness visits.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.
- Ability to pass a national background check and clinical license search.
Preferred
- Experience working in a mobile, virtual, or technology-enabled care setting.
- Comfort using laptops, mobile devices, and HIPAA-secure messaging apps for care coordination.
- Demonstrated problem-solving skills and comfort with last-minute changes.
- Prior experience serving both adult and pediatric patients.
