Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Provide preventive care interventions and patient education on health and well-being.
- Coordinate care with in-home clinical team members to gather vitals, arrange blood draws, and complete patient-specific tasks.
- Document clinical findings and manage care plans in a secure digital workflow.
- Arrange referrals to specialists and community resources as needed.
- Use laptops, mobile devices, and HIPAA-secure messaging tools for charting and care coordination.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
