The Nurse Practitioner conducts virtual wellness visits for adult and/or pediatric patients in the home setting, focusing on health assessments, preventive care, medication review, and cognitive screening. The role also supports care coordination, including referrals, community resources, documentation, and collaboration with in-home clinical team members. Technology is used throughout the workflow for scheduling, charting, and secure care coordination.
Key Responsibilities
- Conduct virtual wellness visits and health assessments for patients in their homes.
- Review medications, perform cognitive screenings, and provide health education.
- Coordinate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Provide preventive care interventions, medication management, and referrals to specialists.
- Document findings and communicate care needs using secure digital tools.
- Use technology to review caseloads, manage schedules, and support care coordination.
- Work independently while maintaining high-quality, patient-centered care.
Required
- Board Certified Family Nurse Practitioner.
- Active Family Nurse Practitioner license in California.
- Credentialed Medicaid provider in California (Medi-Cal).
- Experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
Preferred
- Prior experience in a virtual or mobile care model.
- Strong comfort with laptops, mobile devices, and HIPAA-secure messaging tools.
- Experience working autonomously in a fast-changing environment.
