Conduct virtual wellness visits for adults and/or children in the home setting, focusing on health assessments, risk screening, medication review, and preventive care. Provide additional care coordination, referrals, and documentation while collaborating with in-home team members and using technology to support a mobile, patient-centered workflow. This role emphasizes independent work, strong communication, and compassionate, high-quality service in a tech-enabled care model.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Evaluate vital signs, coordinate blood draws, and support other in-home care tasks with clinical team members.
- Provide preventive care interventions, medication management, and patient education.
- Coordinate referrals to specialists and community resources as needed.
- Document clinical findings and care coordination in secure digital tools.
- Use laptops or mobile devices and HIPAA-secure messaging apps to support scheduling and communication.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in KY, MI, OH, or IL.
- Ability to pass a national background check and valid clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently and in a team environment.
Preferred
- Prior experience performing wellness visits.
- Technologically savvy and comfortable using laptops, mobile devices, and secure messaging tools.
- Experience working in fast-changing settings that require last-minute problem solving.
