Provide virtual wellness visits for adult and pediatric patients in the home setting, with a focus on health assessments, preventive care, medication review, and cognitive screening. This part-time role also includes care coordination, documentation, referrals, and collaboration with in-home clinical team members to support low-acuity, patient-centered care. The work is supported by mobile and HIPAA-secure technology and emphasizes autonomous practice, professionalism, and compassionate communication.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes.
- Provide health risk assessments, medication reviews, and cognitive screenings.
- Coordinate with in-home clinical team members to evaluate vital signs and complete patient-specific tasks.
- Arrange blood draws and support other diagnostic or low-acuity services as needed.
- Provide preventive care interventions, medication management, and patient education.
- Coordinate referrals to specialists and community resources.
- Document clinical findings and communicate through secure digital tools.
Required
- Board certified Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Virginia.
- Experience performing wellness visits.
- Ability to pass a national background check and valid clinical license search.
- Ability to work independently and in a team environment.
Preferred
- Prior experience in mobile, virtual, or home-based care.
- Comfort using laptops, mobile devices, and HIPAA-secure messaging apps.
- Strong problem-solving skills and ability to adapt in a fast-changing setting.
