Conduct virtual wellness visits for adult and pediatric patients receiving care in their homes, with a focus on health risk assessments, medication reviews, cognitive screenings, and preventive education. Support broader care coordination by documenting findings, arranging referrals, and working with in-home clinical team members to complete vital sign checks, blood draws, and other patient-specific tasks.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in the home setting.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Provide patient education related to health and wellness.
- Coordinate with in-home clinical team members to evaluate vital signs, arrange blood draws, and complete patient care tasks.
- Document clinical findings and manage care coordination activities.
- Provide preventive care interventions, medication management, and specialist referrals.
- Use technology for charting, schedule navigation, and HIPAA-secure care coordination.
Required
- Board certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Texas.
- Ability to pass a national background check and clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently and within a team environment.
Preferred
- Prior experience performing wellness visits.
- Comfort with last-minute problem solving and adaptable workflows.
- Technologically savvy and comfortable using laptops, mobile devices, and secure messaging tools.
