Key Responsibilities
- Conduct virtual wellness visits and health assessments for adult and pediatric patients.
- Review medications, perform cognitive screenings, and provide education on health and well-being.
- Coordinate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Provide preventive care interventions, medication management, referrals to specialists, and community resources.
- Document clinical findings and communicate care needs using HIPAA-secure tools.
- Use laptops or mobile devices to manage scheduling, charting, and care coordination workflows.
- Work independently while collaborating with team members to address patient needs.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Ability to pass a national background check and valid clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.
