Provide virtual wellness visits for adult and/or pediatric patients in their homes, with a focus on health assessments, risk screening, medication review, and preventive care. The role also includes care coordination, documentation, and support for related services such as blood draws, vital sign collection, and referrals. This part-time position uses modern technology and involves close collaboration with in-home clinical team members.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in the home setting.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Coordinate care with in-home team members to evaluate vital signs and arrange blood draws.
- Provide preventive care interventions, patient education, and health coaching.
- Manage referrals to specialists and community resources.
- Document clinical findings and care plans using digital tools.
- Use HIPAA-secure messaging and mobile devices to support care coordination.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in South Carolina.
- Experience performing wellness visits.
- Ability to pass a national background check and valid clinical license search.
Preferred
- Prior experience working in a virtual or technology-enabled care model.
- Strong comfort working independently and solving problems in a fast-changing environment.
- Experience supporting both adult and pediatric patient populations.
