The clinician performs virtual wellness visits for adult and pediatric patients, focusing on health assessments, medication reviews, cognitive screening, preventive care, and patient education. The role also includes care coordination, referral support, and documentation while working alongside in-home team members who help facilitate patient visits and related tasks. This is a part-time, tech-enabled role that requires comfort working independently with a home-based, mobile care model.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Provide preventive care interventions and patient education on health and well-being.
- Coordinate care with in-home team members to evaluate vital signs and arrange blood draws.
- Manage referrals to specialists and community resources.
- Document clinical findings and care coordination activities.
- Use laptops, mobile devices, and secure messaging tools for charting and communication.
- Work autonomously while collaborating with clinical team members as needed.
Required
- Board certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Connecticut.
- Experience performing wellness visits.
- Ability to pass a national background check and valid clinical license search.
Preferred
- Prior experience with virtual care or telehealth workflows.
- Strong technology skills and comfort using HIPAA-secure messaging and digital charting tools.
- Experience working in a mobile or home-based care model.
