This part-time nurse practitioner role focuses on virtual wellness visits for adults and/or pediatric patients, with health assessments, medication reviews, cognitive screenings, and preventive care support. The work is patient-centered and mobile-enabled, with collaboration alongside in-home clinical team members to gather vitals, arrange blood draws, coordinate referrals, and document findings. The role also includes care coordination, patient education, and use of secure digital tools for charting and communication.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in a home-based care model.
- Provide health assessments, medication reviews, cognitive screenings, and preventive care interventions.
- Coordinate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Document clinical findings and support care management workflows.
- Use laptops, mobile devices, and HIPAA-secure messaging apps for charting and care coordination.
- Refer patients to specialists and community resources as needed.
- Educate patients on health and wellness topics during visits.
Required
- Board certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Michigan.
- Experience performing wellness visits.
- Ability to pass a national background check and valid clinical license search.
