This role involves conducting virtual wellness visits for adult and pediatric patients in their homes, with a focus on health assessments, medication reviews, cognitive screenings, and preventive care. The clinician also supports care coordination, referrals, and documentation while working alongside in-home team members who handle on-site tasks such as vital signs and blood draws. The practice uses modern technology for scheduling, charting, and HIPAA-secure communication, and expects independent work with strong patient-facing communication and problem-solving skills.
Key Responsibilities
- Conduct virtual wellness visits for adult and pediatric patients in the home setting.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Provide patient education on health, wellness, and preventive care.
- Coordinate care with in-home team members to review vital signs and arrange blood draws.
- Manage referrals to specialists and community resources as needed.
- Document findings and care plans in the charting system.
- Use laptops, mobile devices, and HIPAA-secure messaging tools for scheduling and care coordination.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Oklahoma.
- Ability to pass a national background check and valid clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently and in a team environment.
Preferred
- Prior experience performing wellness visits.
