The Nurse Practitioner conducts virtual wellness visits for adult and pediatric patients, focusing on health risk assessments, medication reviews, cognitive screenings, and patient education. Care is delivered in a home-based, mobile model with support from in-home clinical team members, and may also include preventive care coordination, referrals, and documentation.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in the home setting.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Coordinate with in-home clinical team members to evaluate vital signs, arrange blood draws, and complete patient-specific tasks.
- Provide preventive care interventions, medication management, and patient education.
- Coordinate referrals to specialists and community resources.
- Document clinical findings and care coordination activities using digital tools.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in New York.
- Experience performing wellness visits.
- Ability to pass a national background check and valid clinical license search.
Preferred
- Prior experience working in a mobile or virtual care setting.
- Comfort using laptops, mobile devices, and HIPAA-secure messaging tools.
- Strong written and verbal communication skills.
