This part-time Nurse Practitioner role focuses on virtual wellness visits for adult and pediatric patients in their homes, with an emphasis on health assessments, preventive care, and proactive screening. The work also includes medication review, care coordination, referrals, and documentation while partnering with in-home clinical team members and using mobile technology to manage visits. This is a tech-enabled, home-based care model that values independent work, collaboration, and personalized patient engagement.
Key Responsibilities
- Conduct virtual wellness visits for adult and pediatric patients in the home setting.
- Provide health risk assessments, medication reviews, cognitive screenings, and patient education.
- Coordinate care with in-home clinical team members to support patient needs during visits.
- Evaluate vital signs, arrange blood draws, and help complete necessary in-home tasks.
- Manage preventive care interventions, medication management, and referrals to specialists or community resources.
- Document findings and use mobile technology and secure messaging tools for charting and care coordination.
Required
- Board certified Family Nurse Practitioner.
- Active Family Nurse Practitioner license in North Carolina.
- Ability to pass a national background check and clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.
