Conduct virtual wellness visits for adult and/or pediatric patients in their homes, with a focus on health assessments, medication reviews, cognitive screening, preventive care, and patient education. Work alongside in-home clinical team members to evaluate vital signs, arrange blood draws, coordinate referrals, and document findings. The role uses technology-enabled workflows for charting and secure care coordination, and includes a strong emphasis on autonomous, patient-centered care.
Key Responsibilities
- Conduct virtual wellness visits and health assessments for patients in the home setting.
- Provide medication reviews, cognitive screenings, and preventive care interventions.
- Coordinate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Document clinical findings and manage care coordination through secure digital tools.
- Refer patients to specialists and community resources as appropriate.
- Provide patient education focused on health and well-being.
- Use laptops, mobile devices, and HIPAA-secure messaging apps for charting and communication.
Required
- Board certified Family Nurse Practitioner.
- Active Family Nurse Practitioner license in KY, MI, OH, or IL.
- Prior experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently and collaboratively.

- Experience working in a mobile, technology-enabled care environment.
- Comfort with last-minute problem solving and flexible workflows.
- Experience supporting both adult and pediatric patient populations.
