Key Responsibilities
- Conduct virtual wellness visits and health assessments for patients in the home setting.
- Provide medication reviews, cognitive screenings, and preventive care interventions.
- Coordinate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Document findings and manage care coordination, including referrals to specialists and community resources.
- Use laptops, mobile devices, and HIPAA-secure messaging apps for charting and care coordination.
- Educate patients about their health and well-being during wellness visits.
- Work autonomously while partnering with team members to meet patient-specific needs.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Ability to pass a national background check and valid clinical license search.
- Strong written and verbal communication skills.
