This role focuses on virtual wellness visits for adults and/or children in the home setting, with an emphasis on health assessments, medication review, cognitive screening, and preventive care. The clinician also supports care coordination, referrals, education, and documentation while working with an in-home team that assists with vitals and blood draws. The position uses mobile technology and secure messaging to manage scheduling, charting, and coordination across patient visits.
Key Responsibilities
- Conduct virtual wellness visits for adult and/or pediatric patients in their homes.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Provide patient education on health, wellness, and preventive care.
- Work with in-home team members to evaluate vital signs and arrange blood draws.
- Coordinate care management, including referrals to specialists and community resources.
- Document findings and communicate through HIPAA-secure messaging tools.
- Use laptops or mobile devices to review schedules, chart, and prepare for visits.
Required
- Board certification as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Experience performing wellness visits.
- Ability to pass a national background check and clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently and as part of a team.
Preferred
- Prior experience working in a technology-enabled care environment.
- Comfort with last-minute problem solving and autonomous decision-making.
- Prior experience supporting care coordination or preventive care workflows.
