Key Responsibilities
- Conduct virtual wellness visits for adult and pediatric patients in the home setting.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Coordinate with in-home clinical team members to evaluate vital signs and arrange blood draws.
- Provide preventive care interventions, patient education, and care coordination.
- Manage referrals to specialists and community resources as needed.
- Document clinical findings using laptops, mobile devices, and HIPAA-secure messaging tools.
- Work independently while maintaining high-quality standards and patient service.
Required
- Board certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner license in Louisiana.
- Ability to pass a national background check and valid clinical license search.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.

- Prior experience performing wellness visits.
